Request for Disclosure, etc., of Personal Data in Possession

(1) "Request for Disclosure, etc." to be made to:

The Request for Disclosure, etc. shall be made by sending the specified application form together with necessary documents attached thereto by mail to the following addressee: (It is appreciated if a note in red ink is shown on the envelop to read, "Request form for disclosure, etc. enclosed".)

(2) Documents to be submitted together with the "Request for Disclosure, etc.":

For making the Request for Disclosure, etc., please download the application form (1), fill in all necessary information requested, enclose a document required for the personal identification and send them to the addressee stated in the above Paragraph 1.

(1) Application form specified by the company: "Application for Disclosure, etc., of Personal Information in Hand"
(2) Document for personal identification: A copy of either one of driver's license, residence certificate, passport, etc.

(3) "Request for Disclosure, etc." by a proxy:

When the person to make the Request for Disclosure, etc. is a legal representative of the principal person, minor or wards who are of age or a proxy whom the principal person delegated for making the Request for Disclosure, etc., please enclose the following documents in addition to the documents stated in the preceding paragraph:

(1) "Matters regarding the proxy"
(2) Document to show the proxy's identification: A copy of either one of driver's license, residence certificate, passport, etc.
(3) "Power of Attorney" and certificate of registered seal impression: When the proxy is a legal representative such as the person in paternal authority, it is possible to submit a certified copy of family register, an abstract of family register or a residence certificate, which shows the relation with the principal person, in place of a power of attorney.

(4) Charge required for the "Request for Disclosure, etc.":

Only when a request for disclosure or a request for notification of use purpose regarding the personal information is made, 1,000 yen per request will be charged. Please enclose unused postal stamp(s) worth 1,000 yen at the time of making such request.

* When the charge is in short or when the charge is not enclosed, the company will inform accordingly and, when the charge is not paid within the specified period, the request will be dealt as a non-effective request.

(5) Reply manner to the "Request for Disclosure, etc.":

The reply will be given in writing and sent to the address of the requesting person stated in the request form.

(6) Use purpose of personal information acquired with regard to the "Request for Disclosure, etc.":

The personal information acquired in association with the "Request for Disclosure, etc." shall be dealt within the scope necessary for the request for disclosure, etc. The submitted documents will be sent back together with the reply to the request for disclosure, etc. * Cases when the personal information is not disclosed: In case one of the cases stated below applies, the requested personal information will not be disclosed. When it is decided not to disclose the information, such notification will be made together with the reason of non-disclosure. The specified charge has to be paid even when it is decided not to disclose the information.

• When the address stated on the application form, the address stated on the document to identify the requesting person and the address registered at the company are not identical or otherwise the person requesting the disclosure can not be identified
• When the power of attorney cannot be confirmed in case the request is made by a proxy
• When there is an inadequacy in the documents specified to be submitted
• When the personal information in the company's possession cannot be identified by the statement of the application form
• When the information requested for disclosure does not fall under the "Personal data in hand" specified by the Personal Information Protection Law.
• When the disclosure may possibly do harm to the life, body, property or other right or profit of the principal person
• When the disclosure may possibly disturb a proper conduct of the company's business
• When the disclosure violates other law or regulation

(7) Revision of the procedure:

A part of the contents of this procedure for the request for disclosure, etc. may be revised in order to promote the protection of the customers' personal information and to comply with the revision of relevant laws and regulations. It is requested to confirm this procedure in each case when the request for disclosure, etc., is contemplated.
